Information Collection and Use

Axitus collects two types of information from volunteers: contact information and medical information.

Contact Information

Contact information includes your name, email address and (optionally) phone number. We use this information to contact you in future if you permit us to do so. We also ask you to create a unique user name and password that will allow you to update your medical and contact information in future. We will never give your contact information to a third party without your permission.

Medical Information

Medical information includes details of your medical conditions and treatments. It also includes demographic information such as age, sex, and zip code. This information is essential in helping us understand the usage and effectiveness of different treatments.

Information Use

We group the medical information provided by volunteers to generate data for the purpose of scientific research. Since such information is always aggregated, it cannot be used to identify a specific volunteer. We never provide your personally identifiable medical information to any of our partners without your prior permission.

Log Files

Like most web sites, our servers use log files. Log files help us analyze trends, administer the site, track usage, and gather broad demographic information. The log files are not linked to any personally identifiable information.

Cookies

A cookie is a piece of data stored on a user's computer that is tied to information about the user. We use session cookie to improve your browsing experience. Once you close the browser, the session cookie disappers. Nothing is left on your computer. We do not use cookies to track user traffic patterns.

You can set your browser to warn you before accepting cookies or to block cookies. If you block cookies, you may still use our web site.

Choice/Opt-Out

Our site provides volunteers with the opportunity to opt-out of using their personal information. Volunteers who no longer wish to be contacted by us may opt-out by emailing us at privacy@axitus.com.

Security

We take many precautions to protect our volunteers' information. When you submit sensitive information via our web site, your information is protected both online and off-line. Only employees who need the information to perform a specific job are granted access to personally identifiable information.

Notification of Changes

If we change our privacy policy, we will post those changes in this privacy statement. If we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will notify volunteers. Volunteers will be able to opt out of any new use of their personal information.
